Death Certificate Query

I recently applied for a copy of a death certificate for a relative from the GRO. On perusal of the certificate it give the usual information you would expect including name and address of the informant. However in the box titled Qualification, and where I would have expected to see the relationship of the informant to the deceased, it stated "Causing the body to be cremated".

I must admit this is the first time that I have ever seen such an entry and can only assume that it is possibly an error made by the Registrar. However I was wondering if anyone on the forum has come across something similiar when researching their family history and is it a mistake?
